What a proper rehab-to-performance plan actually looks like

Many runners think rehabilitation ends when the pain disappears.
It doesn't.
Pain settling is important but it’s only the beginning of the process. If the goal is to return to running stronger, faster and with fewer setbacks, rehabilitation needs to extend far beyond symptom management.
The real goal is building your resilience, not just simply reducing pain. That's the difference between managing symptoms and building a runner.
Phase 1: Calm the Symptoms
Every rehabilitation journey starts by reducing pain and settling irritated tissues. This doesn't always mean complete rest, in many cases, runners can continue training in some capacity by adjusting pace, volume or using cross-training options like cycling or swimming.
The objective is to keep you moving while allowing the injured tissue to recover appropriately.
Pain relief is important, but it's not the finish line, it's simply what creates the opportunity to begin rebuilding.
Phase 2: Understand Why the Injury Happened
This is where many rehabilitation plans stop, once their symptoms improve, runners are often told they're ready to gradually return to training. Unfortunately, if nothing has changed physically, the same factors that caused the injury are still there.
That's why every assessment at Equip Rehab looks beyond where it hurts.
We examine your training history, running load, previous injuries, movement quality, joint mobility and strength throughout the entire kinetic chain. We also use objective testing, including force plates and dynamometry, to identify strength deficits, asymmetries and performance limitations that may not be obvious during a standard consultation.
Understanding the "why" allows us to create a plan that addresses the cause, not just the symptoms.
Phase 3: Rebuild Capacity
Once pain is under control and weaknesses have been identified, the focus shifts to building your body's ability to tolerate running again with strength training (Lauersen et al., 2018).
This isn't simply about adding kilometres back into your week.
Your rehabilitation program should include:
Progressively building running volume
Individualised strength program
Plyometrics
Each component is tailored to your assessment findings and your running goals. The strength program should not be generic, they’re prescribed with specific exercises, loads and progressions based on the deficits identified during testing.
Phase 4: Don't Rush the Progress
One of the biggest reasons runners suffer recurring injuries is progressing too quickly. A successful rehabilitation strategy should increase capacity gradually enough that your body can adapt.

Consistency always beats intensity when it comes to long-term progress.
Phase 5: Build Long-Term Resilience
The final stage is often the one that's overlooked, just because you're running again doesn't mean rehab is finished.
Long-term resilience comes from continuing to develop strength, maintaining good training habits and monitoring workload over time. Research and clinical experience consistently show that previous injury is one of the strongest predictors of future injury (Fulton et al., 2014, Kiesel et al., 2014, Rehorst et al., 2025), making ongoing strength training and intelligent programming essential for staying healthy.

A proper rehab-to-performance plan isn't about collecting exercises or waiting for pain to disappear.
It's a structured process that moves from symptom management to rebuilding physical capacity and developing the resilience needed to continue progressing long after rehabilitation has finished.
When every stage is approached with purpose, injury becomes more than something to recover from, it becomes an opportunity to build a more capable runner.
